Temporary and Permanent Supported Housing Officers needed for various Supported Housing Schemes in East London.

The candidate is required to work as part of a team of staff, visit tenants assessed as requiring housing support in order to maintain heir tenancy and/or independent living, provide help and assistance to achieve the best possible outcome for the individual, and provide an emergency response as required

 be responsible for undertaking casework for specific tenants tailored to meet individual support needs by contacting relevant statutory or voluntary agencies and co-ordinating housing support, housing management and health or care services as appropriate

identify and assess and document additional support needs requiring follow on work or individual casework and discuss these within the team to ensure a consistency of approach and to enable the Supported Housing Co-ordinator to effectively utilise team resources

 assess the housing support needs of new and potential tenants, including risk assessment, and update and re-assess the needs of existing tenants to ensure individual suport plans are completed and updated

liaise with appropriate council service teams as required on all maintenance and cleaning of communal rooms and areas and co-ordinate bookings as appropriate
